Printable forms for DOSS
Part of the process for obtaining services is submitting the appropriate paperwork. Students wishing to receive reasonable accommodations may print out the following forms and mail them to David Golias, assistant director of DOSS, Northwestern College, 3003 Snelling Avenue North, Saint Paul, MN 55113, or fax forms to 651-628-2065.
DOSS Program Check-In
- The DOSS Program Check-In provides us with the basic information needed in the DOSS office, and the Data Privacy Clause will give permission for DOSS to communicate with those who will work with you.
Information Release Form
- The Information Release Form is a template letter that DOSS can address to a medical professional if they need proof of your willingness to send pertinent information to our office, here at NWC. DOSS only needs to use this if you cannot provide the DOSS office with the documentation. Typically, the patient can access the documents easier than DOSS can.
